Our reception team

Our reception team are here to assist you. Below is some information relating to the most frequently asked questions about how we run our Practice.


Making an Appointment


We want to make it easy for you to see a doctor, nurse or other healthcare professional without a long wait. We operate timed-appointments only. Our appointment system is designed to ensure that patients can see a member of the practice on the same day that they ask for an appointment. Please note that you may not be able to see the doctor or nurse of your choice on the day. We also retain a limited number of appointments that can be booked in advance.

In order to make an appointment, please telephone our reception line on 01978 760468 early in the morning on the day that you want to be seen. You may also call into reception to make an appointment at any time, but please be aware that you will be attended to in the same way as patients who telephone the reception line.

Our telephone lines are open from 8.30 am, and the majority of morning and afternoon appointments will be kept available for booking on the day. We still have a limited number of appointments that can be pre-booked for those patients that need to make specific arrangements to come to the surgery. 

By restricting the number of pre-bookable appointments in this way, we can ensure that doctors will not be overbooked weeks ahead and therefore should be able to see you on the day that you want to be seen.

Minor Ailments Clinic 

A minor ailments clinic is run by our nursing team every day and will be able to advise on and treat minor injuries, throat infections, ear infections, vaginal discharge/thrush, urine  infections/cystitis, nappy rash, insect bites and stings, hay fever, conjunctivitis, styes, rashes, impetigo, athletes foot and cold sores. Please phone on the day you are ill and need the appointment.  There will always be a doctor available if a prescription is needed, however, if you think that your problem is complicated and will definitely need a doctor’s opinion, please book an appointment in the normal way.

Telephone Advice Line

If you are not sure if you need to see a doctor or nurse, we also run a daily telephone advice line between 1130-1230 (GPs) and 1300-1330 (nurses). As well as providing direct advice to patients & their carers, this helps to reduce the congestion on the phone lines early in the morning. Please note that a queuing system operates and during busy periods.

Nurse-led Clinics

The nursing team provide a number of minor ailment & chronic disease management clinics. A health care assistant is also available for for monitoring areas like blood pressure, ECGs, urine testing, spirometry and basic medic examinations.

Appointment Times

The surgery is open between 0830 and 1830, Monday to Friday. Emergencies only are seen between 1800 and 1830.

Registering with the Practice

Patients who live within the practice boundary (see below) can register with the Practice by calling in at reception.  When registering, please bring your NHS Medical card and birth certificate to the practice and those of any other persons registering with you.  If you do not have a medical card, you will be asked to complete a registration form for each person registering with the practice.  In addition all patients will be requested to complete a new patient questionnaire , allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one. 

If you move out of our practice area, you will need to register with a new practice as soon as you move.  Temporary Residents (e.g. holidaymakers) can register on a temporary basis, again by calling in at reception and completing the necessary forms.

We operate as a group practice, and you may consult with any member of the team. However, should you have a preference for a particular practitioner, please advise us and we will do our best to comply. This may mean having to wait a little longer to see the practitioner of your choice.

Practice Boundary

We accept registrations from patients within the area marked on the map below. This includes the villages of Hope, Caergwrle, Pontbodykin, Leeswood, Pontblyddyn, Treuddyn, Llanfynydd, Ffith, Cymau, Cefn-y-bedd, Abermorddu, HIgher Kinnerton & Llay (part).

Home Visits

We are happy to visit you at home if you are too ill to travel to the surgery, but if you can attend the surgery, please do so. 

In order to assist us in planning our rounds, home visits should be requested by telephoning the reception line on 01978 760468 before 11.00a.m. if possible. 

Please be prepared to give the receptionist some details of your illness, so that we can assess the relative urgency of calls for that day. Please remember that we can see several patients at the surgery in the time it takes to do a home visit.

Out of Hours Care

Outside of surgery hours, primary care services are provided by Flintshire Local Health Board via an on-call doctor service. They may be contacted by telephoning the Practice reception line on 01978 760468 where a recorded message will tell you the out-of-hours telephone number, or by calling direct on 01244 834999. Alternatively you can contact NHS Direct on 0845 4647.

Regular Check-ups

Patients aged between 16 & 74 who have not had a consultation within the last 3 years, may request a check-up.

Patients aged over 75 who have not had a consultation within the last 12 months, may request a check-up. If you are unable to attend the surgery because of your medical condition, a home visit may be arranged.

Non-NHS / Non-GMS Services

Not all services we provide are covered by the NHS. These include services such as medical examinations for insurance or employment purposes or writing letters and reports for non-NHS purposes. Fees are payable by the patient for these services, at the time that the service is provided. A schedule of our fees is available from Reception.

Test Results

Please telephone the reception line on 01978 760468, where our reception staff will be able to pass on test results that are normal or as expected. Should the doctor or nurse wish to discuss the results with you for any reason, then you will be asked to phone back at a convenient time to speak to them, or an appointment may be made.

Dispensing

We are a dispensing practice, with an on-site pharmacy. This means that we may dispense your medications for you if you live more than 1 mile from a community pharmacist. Our computer system will automatically identify if we are allowed to dispense for you.

Repeat Prescriptions

Patients requiring regular medication, may, with a doctors approval, obtain repeat prescriptions by ordering them in person, by Fax or by post.

You prescription may be collected in person, posted to you (if you include a stamped addressed envelope with your request), or sent direct to the local chemist. We are also able to deliver medications direct to patients for whom we dispense (see above).  If you request a neighbour or friend to pick up your prescription or medication it will be assumed that you are happy with them being aware of the information on the prescription or medication label.

Please allow at least 2 working days from when we receive your request, to process it. This ensures we have sufficient time to do the necessary checks before issuing your prescription.

Please note that for reasons of safety, we cannot accept telephone requests for prescriptions (except registered blind patients), nor can we accept requests via email or our website.

Data Protection / Confidentiality

Your personal information is as important to us, as it is to you. We collect and store data in both paper and electronic forms.

You medical records are held in the strictest confidence. Information is not passed on without your consent, unless it is within the confines of the NHS, within a proper legal framework, or is in the wider public interest. Certain anonymised data may be shared for public health, audit, research, teaching or training purposes.

The Practice is registered under the Data Protection Act. It is a Practice and statutory requirement that all staff maintain confidentiality of patients' records.

The Freedom of Information Act 2000 obliges the Practice to produce a Publication Scheme. This is a guide to the classes of information the Practice intends to routinely make available.

Violence & Aggression

In common with the rest of the NHS, we operate a zero-tolerance policy towards any form of aggression or violence towards any member of the practice team, or patient/visitor on our premises. This includes all forms of physical, verbal or threatening behaviour.

Anyone threatening or abusing any member of the team may be removed from the practice list, or other restrictions placed upon the services we provide to them.
